Academic Services Memorandum No. 3

Advising note content is discussed quarterly with the student before the advisor submits the note to the student file in Academic Services (T310, Box 357260). Advising notes include:

  1. name of student
  2. dates, including year and quarter
  3. assessment of student progress in the program and specialty, if applicable. Satisfactory progress is defined by degree program:
  4. advisor name and signature
